We have an exciting new position available for a Qualified Social Worker to join a community mental health team in Essex



Responsibilities:



To observe, record and report any changes in service users’ conditions and circumstances and ensure that they are reported to senior staff without delay.

Any request for a new assessment of need should be reported without delay.

To support service users to identify and promote their needs.

To respect the personal, religious and cultural beliefs of service users and their carers at all times.

To maintain confidentiality at all times in accordance with the policies and procedures

To be aware that any information received while on duty is dealt with appropriately.

To ensure that all records and case files of service users and others for which the post holder is responsible are properly maintained, with full regard to the need to record the views of service users and third parties.

To prepare and submit reports as requested on time and to ensure that all correspondence and communication is in accordance with agreed practices and procedures.

To attend weekly multi-disciplinary meetings to discuss individual care programs and provide information to enable care plan goals to be set.

To ensure that service users are treated with respect and dignity at all times





Pay rate is up to £21 Per Hour



GSCC Registration is essential.
